Passport for Middle School Teachers

The 80-hour Follow-Up (Passport) Program is a district-sponsored activity. Contact your district representative for information related to the submission of Passports.

Submission Information

Download and print the 80-hour Follow-up (Passport) within two years of the completion of the initial professional development institute depending on your district requirements.
Consult your district for passport requirements.
Date and have initialed entries for 80 or more hours of district Passport activities.
Include Passport event documentation, if required by the district. Consult with your site administrator or coach for district guidelines regarding documentation.
Turn in the Passport to the site administrator or designee for certification per the district’s requirements.
